Required documents

You must submit original and two copies of all the documents listed bellow.
All documents must be translated into Spanish.

1.  Visa aplication form. The application form must be signed and filled out in print. (form instructions) 
2. Two passport-type photos: ( see specifications)
3. Passport:Passport valid for a minimum of 4 months after the intended date of departure from Spain, with at least one blank page to affix the visa. 
4. Form EX-00 
5. Non-Us citizens will have to provide Evidence of their Immigration Status in the US (Non-US passport holders only) Provide and “alien Registration Card” or a US Visa with I-20 / IAP-66.
6. Medical certificate : Original translated into Spanish+ 1 copy
   This Certificate must be signed in the hand of the doctor (M.D. or D.O.) in a letter-head paper in the following format:
   “This medical certificate attests that Mr./ Mrs. ……………………… does not suffer from any illness that would pose a threat to public health according to the International Health Regulations of 2005.”
   Visit the web page of the World Health Organization to learn the exact information regarding the control and containment of known risks to public health. http://www.who.int/ihr/health-risks/en/ The certificate must also bear the official stamp of the administering center; however, this will not substitute the absence of the doctor’s signature. Any amendment to this certificate or erasure may render it invalid. This certificate must be issued in the place of residence, and is valid for 3 months counting from the date it has been issued..
7. Certification of “absence of police records”, for 18 year-old applicants and older. Original translated into Spanish and one copy will be needed. This certificate can not be older than three months from the issue date.
   The certification of "absence of police records" should be certified by the police authorities of all places where the applicant has resided during the past five years.
1. In the U.S.,this certificate must be issued by the FBI – Criminal Record History or FBI Identification Report -, It should be verified with a fingerprint card. Process to follow:

2. This certificate must be stamped with the "Apostille of The Hague" . Contact the Secretary of State in the State the document has been issued. http://www.sos.ca.gov/business/notary/authentication.htm
2. If the country where the police record was issued is not the U.S., the certificate must be legalized by the Spanish Consulate in that country or with the "Apostille of The Hague"
8. Proof of travel/health/accident insurance: a letter from your insurance company stating that they will cover 100% ( no deductibles) of the medical expenses with emergency and repatriation services and a minimum coverage of 30.000€ or its equivalent in dollars. Policies with reimbursments or co-payments will not be accepted. You can buy travel insurance from your travel agency, through the internet, or an insurance company in Spain.
9. Proof of sufficient funds. You must provide evidence of annual income while living in Spain. Also you should provide evidence of investments or savings to support your application.
10. Proof of ownership, leasing, or renting property in Spain.

12. Spouse of the main applicant  should submit the same documents plus the  Marriage certificate authenticated with the "Apostille of The Hague"  and translated into Spanish.
13. Children of the main applicant  should submit the same documents plus the Birth certificate authenticated with the "Apostille of The Hague"  and translated into Spanish.

Specific requirements: If your residence in Spain is motivated for one of these reasons, you must add  the following documents:

1. Investor/self-employee:  
   1. A complete business plan including information about the investment, market studies, business prospects, possible job offerings, total of money to be invested, projected proffits you wish to execute , etc.
   2. Bank certification: You have to be in possession of 30%, or more, of the total amount of the investment.

2. Retirement :
   1. Proof of any other source of income and/or properties in Spain.
   2. Proof of medical insurance coverage in Spain.

3. Specialized technicians, faculty, researchers, and scientists invited or employed by a Spanish Public Administration or Spanish University:
1. Original official invitation or document describing the activities to be performed, stating also that the organization will assume financial and medical responsability for the applicant during his/her stay in Spain.

4. Administrators and faculty of educational or cultural institutions who will participate in cultural and educational programs in Spain:
1. Original certificate from the educational or cultural institution in Spain describing the activities to be performed, stating also that the organization will assume financial and medical responsability for the applicant during his/ her stay in Spain.
2. College Degree and credentials proving the activity you will be performing in Spain.

5. Public and civil servants or military personnel participating in those activities in Spain under bilateral agreements of cooperation:
1. Original official invitation.
2. Documents describing the activities to be performed.
3. Proof that the organization will assume financial and medical responsability for the applicant during his/her stay in Spain.

6.  News correspondent of a social foreign media:
1. Credentials demonstrating the activity you will be performing in Spain.
2. Proof that the organization will assume financial and medical responsability for the applicant during his/her stay in Spain.

7. Members of international scientific missions:
1. Original official invitation or documents describing the activities to be performed, stating also that the organization will assume total financial and medical responsability for the applicant during his/ her stay in Spain.

8. Artists who will perform in Spain for no more than five consecutive days or twenty days in a six month period:
1. Original documents describing the activities to be performed and dates. Proof that the organization will assume total financial and medical responsability for the applicantduring his/her stay in Spain.

9. Ministers, members of a church and representatives of the different churches or confessions:
1. Letter from the religious organization stating that you have been sent as missionary/ other.
2. Letter from the religious organization in Spain. Certification that the religious organization or church is duly registered in Spain.
3. Proof that the organization will assume financial and medical responsability for the applicant.

10. Representatives or staff of a reconized International Union:
1. Original official invitation or documents describing the activities to be performed, stating also that the organization will assume total financial and medical responsability for the applicant during his/her stay in Spain.

11. Citizens of Spanish origen who have lost their Spanish citizenship:
    1. Certificate of naturalization if you have adquired a foreign nationality.
    2. Certificado Literal de Nacimiento expedido por el Registro Civil Español (Birth Certificate issued by the County Clerk Recorder of Spain)

12. Minors under the legal custody of a Minor Protection Agency:
    1. Documento Judicial de Tutela, (Judicial document of custody).

You will be contacted once your visa is authorized and you (and all your family members applying for a visa) will be required to bring your passport to this Consulate General in order to obtain the visa.
